The most recent and currently one of the rarest, is this Space Flight Awareness Team Award featuring metal flown onboard the Space shuttle Discovery's flight STS-114 in 26/July - 9/August 2005.
The medal presented to members of the launch and assessment teams at NASA, who were involved in the mission. This medal is not made available to the general public.

Soviet "Энергиa" Energija Maiden Launch

This extremely rare medal was minted from the flown metal of Energija's first test flight. This heavy 100 tones rocket was also used as Buran's booster system. Front side of this medal depicts Energia on launch pad and reads "15 May 1987, 21:30 pm, Cosmodrome Baikonur." On reverse side indicated "This medal is minted from original metal of rocket-carrier Energia."

Another extremely rare medal which was minted from the flown metal of Buran's first  flight. The medal depicks on the back an holographic insignia. Buran just flew one time. In 2004 Buran was detroyed after the disruption of the hangar, Buran was stored in.
